I am involved in the development of a regional HIE- as a result I have discussed EHR's with many users- and remain exposed to many interested parties. Soapware has been, by far, the most flexible and highly developed EHR that is easily integrated into small to medium sized practice environments.

Does it Improve the Quality of Care
It has clearly impacted the quallity of care- it facilitates the implementation of guidelines and the monitoring of patient response to therapy as well as preventative healthcare-monitoring.
It is also an amazing teaching tool.
Pros
Very flexible: use/implementation can be at whatever speed is most comfortable. The depth of implementation can be accomplished in a stepwise fashion. New staff can be rapidly trained-due to the online training features. Increase integration with addition of modules. Rapid online help response-and a very knowledgeable staff.
Cons
The only con I see is related to the degree of flexibility-the smart text-and template features- are somewhat complicated- however it is this feature that makes this the most flexible EHR I've worked with

Full EHR with all the needed parts.
Cons
Horribly complicated templates. Impossible for us to customize to our satisfaction. We found them so bad that we've abandoned the system. We complained to the company and got little assistance.
Must use Xlink to tie in with billing software.

Pros
Very robust EHR for not a lot of money. This is a very sophisticated program, and once you understand the degree of structured data input which is possible with it, you understand why it will be the leader in "Meaningful Use" within the field. There is a great deal of ability to customize the program to meet specific needs. The program is designed to evolve with time and meet the changing standards of 'meaningful use', etc. The technical support team is a pleasure to work with - very attentive, able to stream into your system so you can demonstrate any problem you may be having, and usually can solve the problem immediately. There is also a very active user forum website, with many users who are extremely familiar with the program and eager to assist each other with 'learning the ropes'. The company website also has several videos demonstrating particular aspects of the program, and they have a Wiki site (SOAPedia) which gives extensive documentation of how to implement and use SOAPWare to its potential. Also, the company is extremely receptive to customer feedback on how to improve the program, and they actually respond to our suggestions and requests!
Cons
There are several features which are still actively being developed which still require better documentation as to how to use them.

Pros
Great value, fairly easy to learn, lot of very customizable features.
We expect this product to help us meet 'medical home' requirements and 'meaningful use' requirements.
Very helpful training staff and support.
Cons
This is probably not the easiest product to get started on only because you need to be able to do a lot of the customization yourself. But for those with an interest and the time to prepare, it provides a great product at a very reasonable price. It is probably not the best software for someone who expects everything to run to their specifications 'right out of the box'.
In some ways the product is still under development, some features not quite 'ready for prime time'
